When and why was London designated as the capital of England? And likewise, back in the day, how did they go about doing this in other countries around the world, i.e. why Madrid in Spain, etc?

The usual reason is because it's where the monarch set up shop.
I believe Oxford got off lightly in WW2 because Hitler intended to make it the capital when he ruled the UK
