Party shops have these - some Birthdays stores have them to hire. Or little independant shops, look under party planners in the Yellow Pages for your area, they should have one or be able to point you in the right direction

Also check under 'Theatrical supplies' in Yellow Pages. Any company which mentions stage lighting in their services will hire these to you. (NB: If in doubt, go for smoke, rather than dry ice. The latter can be dangerous if not properly handled).
