Permission From Facebook To Ask Health-Related Questions

I have created an Instant Form on Facebook that contains multiple choice question relating to anxiety (I'm a therapist specialising in helping people with anxiety.)
However, the ad was rejected because I breached the policy with this wording: "Ads must not request health information, including physical health, mental health, medical treatments, medical conditions or disabilities without our prior permission."
It's the part that reads "without our prior permission" that's got me wondering... how do you get that permission?

They're never gonna give it for an ad??? The answer to that is right in the policy... Of course they would with "prior permission."
I found out that they give this permission to, for example, insurance companies. However, I do not qualify.
Oh, and by the way, I have anywhere from 2 to 5 people a day completing these questionnaires in my off-Facebook ads. People do want help and do answer questions such as "Do you avoid places or social situations because of stress/anxiety." Whis was all I wanted to ask in my ad by way.
Facebook is just so precious about jeopardising their ad revenue that you can hardly say anything on there anymore.
