How To Make An Online Reference Link?

As somebody who employs a lot of people on short-term contracts, I spend a lot of time chasing up references. Ideally, I'd like to be able to provide people with a link which they can click on which will lead them to an online reference form. The reference could be mainly check boxes, but would need a space for comments.
Is there software somewhere that will do this? Is it possible?

Hey there,
You can add your referral link at the bottom of the content. Whenever you use the third-party link as a reference you need to mention their link as a reference link. Such as you to use an image of some of the companies you need to mention their original link below it.
