For how long has 'yes' and 'no' been physicalised with a nod of the head or a shake? And why have these actions been attributed to the words? Also is it a world-round gesture?

Don't know why we do it, but it isn't universal. Bulgarians shake their head for 'yes' and nod for 'no'.
