Does anyone know the address to submit the claim for employment & support form to?

Its the ESA (Y) 22 04/11.

It normally comes with a pre-addressed envelope but none this time.

goes to a web page which allows you to find the address of your local Jobcentre Plus office. So that appears to be where you should send the form to.
