I once asked a milkman if he sold milk!  It was the wee hours after a late night and what I meant was had he any extras he could sell.  Did feel a bit of a div at the time!

Back in 1974 when I worked in a record shop, we had a double album in of Fred Astaire's songs from his movies.  An old dear came in and looked at it for a few minutes then looked at me and said, "Does he dance on this dear?".  I shook my head and replied, "Sorry, I've not heard it myself so I don't know." (Doh)
Mine is actually a case of me asking the stupid question...in my defense, however, it seems to be a typical reaction:  someone asks you for directions, as in "how do I get to Woodward Avenue?" and you immediately respond "from here?"  (Which is what I did in this case.)  As the comedian George Carlin once pointed out: "From here?"  "No, from Pago Pago!!"

My wife works in a gift shop , she was once asked if the Union Jack flags came in any other colours.Working in a pub I was once asked what was in the wild boar and pheasant pies that we had on the bar menu.

When I worked in a travel agent, I once got asked if the cabin with balcony had a sea view by the customer on her cruise ship!

Working in a petrol station we rang out of diesel on pump 4.  I told the customer we had some in pump 1.  Which one is that?  The one with a number 1 on it. The other one is when you tell someone that someone you know is being buried/cremated.  Why is he/she dead?

A woman I know heard Beethoven's "Emperor" piano concerto and asked if it was Beethoven playing it.
I once ordered fries in Macdonalds, only to be asked "do you want fries with that"?
