Promoted/relegated teams

When Bolton,,Barnsley, C Palace were promoted to Prem League - all 3 were relegated the following season
Also ,Fulham ,Blackburn , Bolton -promoted to Prem League - all survived
So have there been any other instances of promoted/relegated teams all having the same fate the following season?(this can be promoted -relegated vice versa)

So it's a rather rare event!- must have happened prior to this though i reckon
