Just stuck on two - 16. Supportive equipment with a throat problem. 40. Smooth surface that may be in agony. Also, unsure on this one - This could be painful if played incorrectly. (Wondered SORE and SAW?) Any other ideas?

Thanks very much both of you, I was a bit dubious about HORSE to start with but then thought of (clothes) HORSE so think it's right! Thanks again.
I've put sore saw all though that isnt quite the same as the other clues
