How do I remove an app from my phone ?

What sort of phone?
Varies. On mine one goes into 'apps', long press on the relevant icon until the menu appears, then choose uninstall. On my last phone I had to opt for uninstall prior to selecting the app icon. Play around, see what works.

Try holding it down, if a little X appears, click it. Or, go to settings, applications, then see if you can delete it from there
Go to settings, apps/applications scroll down to the app you want to delete, click it, then click uninstall
If it is an app that came with the phone...
Select Force stop and then Disable — you won't be able to delete the app, but it will stop running on your phone, never to be seen again, until you do a factory reset

not on my android, if you force stop and disable it will re-enable if you click on the app again if you want it back again
Or... you can find the app in the Play Store (assuming that's where it was loaded from) and likely there's an uninstall button there.
google play apps are easy to remove. i think the questionnaire is referring to pre-installed apps gezzer
