I've solved most of this puzzle, including 6D, but have a mismatch of seemingly sensible answers for 7D, 10D and 12A. Any tips would be greatly appreciated.

Don't really understand your question - they're all regular answers. One of yours (12a I'm guessing) must be wrong. Old version of a modern word with first two letters missing.

I have another question. I have found appropriate number of items
in the grid but one can appear in two different ways!
Does anyone have the same problem? There are so many possibilities one could search for, perhaps I've missed one?
