The phrase has been in use for the past three hundred years. It generally suggests, nowadays, that the pregnancy was 'accidental' in much the same way as any other 'fall' is accidental. I suppose the idea is that the condition dropped upon the woman concerned 'like a bolt from the blue'...as if she herself had little to do with it!

Quizmonster is right. It alweays makes me cringe when I hear it. Nobody falls pregnant despite the phrase.
