The phrase refers to not wearing underwear. Its origin - I remember listening to an interview with the friends cast (RE: The One Where No-one's Ready, Season 3). I think it was made up while they were filming the episode. But who knows?

it does refer to wearing no underwear, but it wasn't made up on the friends set. it's because the guys in the army often don't wear underwear when they're fighting.
