How should you address a Dame?

When writting to a Dame how does one address her .... as in Dear ........... and how does one end the letter, as in Yours ...........

'Dear Dame ------- '

'Yours truly,'
or
'Yours faithfully,'
or
'Yours sincerely,'
With the postcode of course (you're not properly addressed without it). :-D
according to Whittakers Almanac, on the envelope should be Dame forename surname, and a formal letter should start Dear Madam, but a social letter should start Dear Dame forename...the letter should end Yours Sincerely...Yours Faithfully should only be used to someone you are personally involved with, a friend, relative, boss etc
