My mother in law has been told she's had a silent heart attack recently. In laymans terms what is it?

Some. fairly rare "heart attacks" can be nearly pain free... hence the term "silent"...

Im assuming your Mother is Diabetic.
A Silent Heart attack is due to the sensation of pain being muted by the effects of diabetes. In Diabetics, the blood carries too much glucose (untreated) which causes peripheral nerve damage, thus lowering the sensation of pain.
Keep it in check.
