I enjoyed it but can anyone explain the order of the way the plot was being unfurled.

I've seen the film recently, but am unsure what exactly you are looking for?
The crux of the film is the guy is insane and has multiple identies. The psychoanalyst states that the purpose of the "interrogation" in the court room (in 'reality', as it were) is to bring out and destroy the multitude of personalities within the subject. Conflicts take place within this group (his mind) with identities slowly being destroyed throughout the film and narrowing down to the ones who actually may have killed those people in the hotel (in 'reality'). 10 down to 1, or so we think, as we assume the little boy has died when in fact he's the one causing all the pain. I suppose the order of death is determined by those with the weakest will dying first, as they'd conceivably be easiest to push from the psyche.

I understood that bit requiem but the sequence of events in the film did not tie up (i think?). Was alfred molina narrating the story to the lawyers/judge etc. as the motel scene started well before Mr serial killer got wheeled in a la hannibal.
