Management speak devised to make HR personnel look clever whilst actually being entirely stupid.
It is based around the idea that skills can be taught but behaviours cannot. Rather than look at your skills or results with regard your job, your employer will look at your behaviours in certain situations. Many job interviews are now competency based now where an interviewer will ask- "Tell me about a time when..." and ask you to explain something you have actually achieved (and how you achieved it.

Move this over to performance management and we are looking at your line manager speaking to you about how you handled situations, customers, colleagues to ascertain your ability to continue in the job, gain promotion or pay rises, etc. All this will be put into the context of your results. E.g - if you are in a sales job then they may talk about how you deal with customers (e.g. telephone manner) in relation to how many sales you have.

As with most performance management there should be no suprises - no-one should be sitting you down in a review and discussing things with you that have not been discussed before. Again E.g. you should not have an annual review where your manager says you have a problem with being late if you have gone through the year and it hasn't been mentioned before.
