Whenever I've done it you pick a name out of a hat (work colleagues usually) and buy for that specific person but they are never supposed to know who they got their present from. Usually there's a money limit on what to spend.

I am working at present with a group of people who hardly have any contact with each other. So this in my mind would be pointless.
