Pot holes in the road

I need some help please with drafting a Small Claims to take the local Council to court for damage to my car sustained through their failure to repair a road either to a satisfactory standard or with sufficient frequency. Does anybody know the actual wording I should use please?

Make sure you take a photo of the road as councils are notorious for filling in potholes before the case comes to court and showing pictures of a tarmacked road with no potholes etc.
