Please can some one help me, A member of my family has had to have a Doctors appointment which they have agreed to make time up for at work. They also have a hospital appointment which they have booked leave for, work has them to provide a letter about the appointment and asked for details why the appointment was needed. what are their rights? They do not want to disclose the reason at the moment until things have been confirmed.

If the employer is giving them the time off, they have the right to ask. They probably want a copy of the appointmenrt letter to prove its a genuine appointment.

If its a private matter, I think what your family member should do is tell photocopy the letter, blacking out any bits that make reference to what the appointments about and then take this to the employer, explaining that for the time being they'd like to keep it private.

If the company won't accept that (which they're within their rights to do) your family member may have to book it as holiday.
it is not clear in your question which appointment the employer want the info on - the doctor's one or the hospital one. If hospital, then the employer has no right to the information as your family member has booked annual leave for this and therefore shouldn't provide info.
