Hertzberg did an early study into human motivation. He theorised that satisfiers were Achievement, Recognition, the Work itself, Responsibility and Advancement. He also said that key dissatisfiers were Company policy and administration, Supervision, Salary, Interpersonal relationships and Working conditions.
A Hertzberg questionnaire presumably seeks to understand this for employees of an organisation - I've never seen one used in this way.
