How do clothes measurements relate to clothes sizes?

So if you have a garment and measure it, what would it say on the label if it had one? I was told that a jacket for example you take off 4" - so if it measure 48" it is to fit a 44" chest. Is that true?
What about trousers? If you measure the waist, how does that relate to the label?
If anyone can give me a link to a site with lots of examples I'd be delighted as I've been looking for ages. Thanks.
