This question was asked before (I just did a search on previous threads) but nobody's heard of a form with that number. Ask the Halifax what it is.

I will be able to call them Monday , but fuming with the Halifax and solicitor for only bringing this up at the 11th hour on a Friday, stuck with the in laws for another weekend!
What is rather odd is that a Google search for 'GM86 form' (with no reference to 'Halifax' in the search) leads to this page:
http://www.halifax-intermediaries.co.uk/literature/mortgages/default.aspx
(but there's no reference to 'GM86' on it!)

Since that page only provide downloads of Halifax's own forms, it suggests that 'GM86' is simply a reference number given to one of their own documents (rather than any 'official' form used by say, RIBA) and, further, that the form either no longer exists or has simply been renamed to something easier to understand.

Regrettably, a phone call to the Halifax looks like your only answer.
