im a father and my child was born in june 2003 my name is on the birth certificate do i still need to fill in the form and what is it for

It depends if you are married. If you are then you don't need to.
I am assuming you aren't though. If you want full parental rights, you need to apply for parental responsibility as you before child was before Dec 2003.
Legally you have no right to make decisions for your child related to any potential medical treatment, education etc., or if the partnership splits up, you have no rights.
With a PRA, you have equal rights to the mother.
Children born after this date are unaffected as the law changed to recognise unmarried fathers as equal to the mothers.
