You don't say whether it's for an adult or a child. The following are all illustrated books, ostensibly for children but may give comfort to an adult. "The Velveteen Rabbit" by Margery Williams should be easy to find, "I'll Always Love You" by Hans Wilhelm may be a bit harder to track down, but is also good. I've not read "Badger's Parting Gifts" by Susan Varley, but I'm told that it is very good. I actually own the others *blush*.
How about "High Flight", by John Gillespie Magee Jr, or "Poem to a Dead Pigeon", by beat poet Jack Micheline. Both are easily Googled.
