In a restaurant in Tucson, one my very favourite cities, we ordered a salmon and a steak meal. We were given the inevitable salad first, even though we asked for it with the meals, and also a huge pile of bread and butter, about 8 thick buttered slices. At the end of the meal our waitress asked if we hadn't liked the untouched bread, apparently other customers usually asked for more!
Wasn't it Reagan who classified ketchup as a vegetable in school lunches, a bit like one of your 5 a day?
