Puzzle Monthly Collection, no.5

Can anyone help with clue20 in Clockwork ? 'Clean and healthy' (8). And what do you all think of Transquotation ? I think they've gone mad there, and I just gave up on it.

interstitial cystitis

My cat has this nasty condition from time to time. He has one capsule of Cystaid a day as a preventative measure, and drinks plenty of water. Does anyone have any useful advice, please ?
