We know that new stars are constantly being formed but is there any evidence that new galaxies are being forms

Short answer... yes. But that's only if you consider any formation within the last 100 million to 1 billion years or so as "new". There are 30 or so that fir in that time frame.
Of the four Hubble Class types (lenticular, spiral, elliptical or irregular) of galaxies, the irregular type may still be forming (there are several sub-types within the category)...

On going velocity and gravitational measurements of stars within the Virgo Cluster seem to indicate formation of a new galaxy... but give it another 100 million years or so to offer proof of the theory...
In 3 billion years' time, our galaxy will merge with Andromeda. I can't wait!
Actually Milky Way and Andromeda will pass through each other a couple of times before the merger.

Given that life as we know it will be impossible on Earth in a billion years, none of us will be able to wait (here at least.)
Why, what's going to happen in a billion years? The sun still has about 5bn years of fuel left.
