If an unpaid volunteer is driving a vehicle owned by a charity and has an accident who should pay for the repair? The volunteer that has already given their time etc to a worthwhile cause or the charity who choose what the excess on the vehicle insurance is?

The charity. Otherwise they'll find themselves with no volunteer drivers.

If the volunteer was at fault they probably ought to offer to meet at least some of the cost on moral grounds
Definitely the charity.
If you are driving a vehicle that is a company vehicle you should have insurance that covers you for driving such a vehicle and not just social domaestic and commuting to and from a fixed place of work, the insurance company can refse payment if it feels the policy was not correct for buisness use.
