If you have a leasehold property and change the inside; so for instance move what was the kitchen to the room identified as the 2nd bedroom - is it usual/customary for permission/authorisation to be required from the leaseholder in order to approve the plans for the potential re-arrangement?

The leaseholder needs to check the terms of the contract. I would certainly advise discussing any changes with the freeholder before making any changes.
