In what circumstances can a final written warning be given if that individual had no verbal or 1st warning before.

Serious misconduct - assaulting a colleague, stealing from the employer or colleagues, ignoring express instructions and putting yourself or others in danger of injury (removing a safety guard for example).

Final written warning as a first measure is used when the employee has committed a serious breach - drunk in the workplace, for example.
