I had a swollen foot which was gradually getting better after ice packs and light physio over the last few months It was diagnosed as strained metatarsals.

But yesterday the physiotherapist gave me an ultrasound treatment on the top of the foot and today I can hardly get my shoe on again. Is it related or just a co-incidence? I'm back to square one and to where I was about 6 months ago.

I would suggest that there is a direct relationship between your swollen foot and the treatment, as initially there may well be an inflammatory (warming) response to ultrasound.
This should be less evident after successive treatments.
