The most popular theory is that Lord (Robert) Salisbury, the Prime Minister in the late 1800s, gave his nephew (somebody Balfour, I think) three different very prominent posts in the government - Chief Secretary for Ireland, President of the Local Government Board and Secretary of State for Scotland. The nepotism was obvious to all, hence the saying.
