Yes, one for everyone.

I would say you would need one for each person who is travelling including children. Also advisable to take out private insurance as EHIC only covers the essential treatment.

You should apply for an EHIC in your name and include your child(ren) as your dependant(s). You will each receive your own EHIC.

if you have one then you know the procedure, but as a sort of warning, don't go on a site if it says you need to pay, as you don't, they are free

Yes you do need for everyone as all have a different NHS number which gets allocated to each EHIC card
