Request Hearing For Back Child Support

I need help! I have to write a (request for a hearing) on back child support, to reopen the case, incarserated at the time of court for dna, so since i wasnt there they declared me the father and now im 2 thousand dollars behind and their wanting to suspend my license. Can someone help on what i should put on this letter? Or how to write it?
