Inability to adjust vision (focus) to near and far would be my guess?

Unlike bi focals which have only two distances, close up for reading at the bottom, and distance in the rest of lens.
I have varifocal lens, which have three distances graded into the lens.
Close up for reading at the bottom. Middle distance in the middle, which I look through for something a few feet away, and far distance, which is the top part of the lens.
Maybe this is what your wife needs to be able to see everything clearly.
They take a bit of getting used to, because you have to adjust your head as well as your eyes. But after a while you dont even notice that you are doing it, and they are much better than bi focals.
