Depends ...if its a bone only one pj bottoms with fabric ties are possibly ok otherwise a hospital gown for soft tissue films ideally nothing

no metallic studs but usually they will pop you in a gown anyway!

The only ones who have to wear anything different is the operator as they are constantly exposed. Last time I had an X-Ray the lady went and behind a special panel

Forgot chest xray is strip to the waist only so it depends what bit is being domne

Clean underpants. you might have to remove your trousers in case zip buttons in the way.
