To lie or not to lie?

If someone asks you if they look fat in something, and you think they do, should you tell them? I have a friend how is fat (hey, everyone needs a fat friend!) and she is not really attractive either but when we go out she asks if things look good on her and most of the time I just say yes but another friend of mine said I am being crueler by not telling her she looks a mess, but surely if she looks a state it's her problem, not mine?
