in laymans terms its knackered mate. is it worth fixing is a subjective question. i have been in the position a few times where i could'nt afford not to. an automatic box is always expensive to repair, just can't be done on the cheap. the amount you would lay out to buy a decent second hand gearbox might just buy you another motor to run around in. have you thought about looking for an mot failure to break for the parts you need?
i advise you to get a new car because the car is going to give up on you sometime and the car is pretty old, so go out and buy a new small car for a cheap price and trade in your old car.
Sounds like a torque converter problem. As to fixing it, it really depends on whether you can get a car in better mechanical order than your's when its been repaired for the same amount of money as you can get for your car in its present state plus the cost of a replacement part. That is if you are on a tight budget and couldn't afford anything significantly better. If you can't, and are not too well up on cars you may end up having to pay out for repairs on your new purchase.
