Hubby bought me this necklace ages ago. Nothing fancy. A heart on a chain. First time I wore it I developed this red raw rash exactly where the chain lay on my neck. Didn’t say anything as didn’t want to offend him. Put it back on yesterday and again have woken up to this red, raw, really painful rash along the exact line of where the chain has laid. Without knowing what it’s made of what’s the likeliest thing I’ve reacted to?
Sounds like Nickel. I have allergies to metals and nickel is one that really sets my skin off badly. Try something in silver or white gold and if that still sets your skin off try titanium or platinum. I can only wear titanium as its the only metal that doesnt react with my skin.

My first though reading your OP was Brass... which contains zinc which in turn can cause a skin-rash as well as the copper.
