when I try to use disk clean up there are literally millions of cookies - what would happen if I deleted them

Nothing much. Any web sites (like AB for example) where you'd elected to be logged in automatically would no longer recognise you, so you'd have to log in manually. Similarly, many sites for which you had set preferences would no longer remember your preferences.
