They were at a soft ball/base ball game and she fell from the stand, can't remember if Homer had something to do with it.

It wasn't baseball - they were at the local car racing track and homer asked the promotions girls to shoot a t-shirt to him - they were using air powered canons - the t-shirt hit Maude and knocked over the top of the grandstand and she fell to her death

Indeed, and Homer also parked in the ambulance bay to prevent the paramedics reaching her. Bit of a continuity error here - they are hiking in the woods when they stumble upon the racing track, they don't drive there!
