Would a bank statement constitute as proof of your address?

Along with something to prove your identity, yes, a recent bank statement should be enough to prove your address. Without the proof of identity (Photographic ID such as passport) you cannot prove it is YOUR address.
also a gas or electric bill
Any current formal notification, eg bank statement, utility bill, which has your name and address on it, plus photographic id (passport, driving licence) is reasonable proof of identity.
Yes it will be normally be accepted as proof of your address.

You will need something else to prove who you are though if that is required.
