Application Form for Section 118 Highways Act 1980 extinguishment order

I have a car parking space that is technically designated as adopted public highway even though my deeds from Land Regitry quite clearly shoe has that parking space is allocated to my apartment. Basically somebody during the redevelopment process 6 years ago didn't do the extinguishment bit on 2 of the car parking spaces.
I have been told that it would be quite in order to get the area extinguished and that there would not be any objections.
I cannot find an application form on the web to begin the process. Any help please.
The highways agency concerned are Plymouth City Council(PCC)I

why don't you just ring their planning or highways dept and ask them?

It isn't that simple as I wanted to read all the info concerned.
they still might be able to help you though, and point you in the right direction, or even give you an idea if it's something they would entertain

I have now found what I was looking for. I put extinguishment application into the guidance notew and voila.
