how do you go about handing over your house to your children , and still living there ,so as to not have to sell should you have to go into care at a later date

Basically you can't.

And you land them with later tax bills if you do.

If you SELL it to them at market price and then pay them a commercial rent to live there then it can't be sold for care home fees.
