I'm sure I've seen kitchen units that appear to be a full cupboard but when you open the door there are drawers inside flush with the front of the unit carcass with half moon cutouts to pull them out with. Can't remember where I saw them though, pop down to Magnet and MFI and the like, they're sure to know of something.

You could hang a cutlery basket from the dishwasher inside the cupboard door.
OR You can buy a revolving stand where knives, forks and spoons are hung which you just take to the table.
Most modern kitchen drawers come flat packed and make up into a box construction with runners supplied, the fronts come as a seperate item. If you have a 500mm wide base cupboard for example, put a 500mm draw box at the top but minus the decorative front, the existing door will then close over the draw when in the closed position. It might be an idea to drill a finger pull hole so that you can open it.
