There are serious flaws that allow even individuals to create havoc with systems that are so difficult and costly to handle.

For instance with computer code this can be modified by an intruder. If this code had been hardwired on a ROM chip it could not be altered although modifications would be more difficult.

Should we go back to the drawing board and design our systems differently?

Whatever systems you have, if access codes are stolen then intruders will be able to enter.

And although there are ways of improving system security, the greatest vulnerabilities are by social engineering and I suspect this may be at the heart of the problem.
