Do you mean how should you write something which explains how the person should go about answering the questionnaire?

follow this...

- Outline the people you are targeting to prevent unwanted mass

-What you're hoping to achieve from the questionnaire so they make their answers relevant.

- How to answer it i.e. circle/delete the relevant answer

- How to return it to you and the deadline for all submissions
