I gave up smoking(successfully until now) by using nicotine patches(step 1) When I tried to go to step 2 the need to smoke was overwhelming and I went back to stage1. The nurse now tells me that I can no longer use step1 and must go to step2. She says that continued use of step1 can be detrimental but cannot tell me how or what the symptoms might be. Can any of you please?

Try cutting the patches in half until you can wean yourself off them
Get yourself some willpower
I stopped smoking using Niquitine 4mg lozenges. I'm still addicted to nicotine, but ,with the tablets, am getting it in a much less harmful way than smoking.
The patches would be doing you less harm than smoking. Ask your GP about continuing to use them.
