Tattoo removal... NOT laser treatment

How would I go about finding out about the type of tattoo removal where they actually slice your skin away?  I think maybe they do a skin graft too?

Would I need to go to a cosmetic surgeon about this or could a doctor help?

Has anyone had this done?  Do you know a rough idea of cost?
